 | MEDICAL EMERGENCIES: For emergencies day or night, call (513) 221-3800. If you call our offices after office hours, you will be referred to our 24-hour answering service. The answering service will promptly notify the physician on-call, who will contact you as soon as possible. |
 | APPOINTMENT SCHEDULING: To schedule an appointment at any of our offices, simply call (513) 221-3800 between 8:00 a.m. and 4:30 p.m. For your convenience, we have flexible hours to accommodate your busy schedule.  | UNABLE TO KEEP YOUR APPOINTMENT? Kindly give our office a 24-hour notice if you are unable to keep your appointment. To cancel your appointment, simply call (513) 682-4506 any time, day or night. Failure to give a 24-hour notice will result in a $25 charge. |
 | ADDITIONAL OFFICE CHARGES: Additional office charges will be assesed for the following occurances: Additional charge for co-payment not paid at the time of service $30, disability form completion: first form $25, second form $20, each additional form $15, and surgery cancelled within 72 hours of procedure $150. |
